perfecttour on "[Plugin: Jetpack by WordPress.com] Contact form missing fields"

I wanted to add the form to one of the pages, but only the field "name" appears in the page and the button "send".
The form is sending correctly.
I tried to add the fotm to pages and widgets, but the problem is the same.
